Before proceeding with a hardware replacement please try to download the latest EXOS 15.3.5.2-patch1-13 version from firmware download portal and then try the following steps to avoid software related issues:

This continous unexpected reboot is likely caused by a H/W issue on the switch, as referenced in the following lines:

STM: bcm_rx_register for unit 1 failed!
STM: bcm_linkscan_register failed for unit 1!

Please open a case with GTAC to help address this for you.
