i receive those logs on our switch one time every month & for very short period (less than 1 min), how can i investigate further & what action should i do ?

Appears that fan#5 is fluctuating off and on. This could be a failed fan overall. To rule out other possibilities, try power cycling the switch/run extended diagnostics, ensure the air vents are clear, and ensure the code is on 15.3 latest. If the above is complete and the issue recurs, raise a case with GTAC for further action.
Thanks 🙂
