I realize this solution may have limited application but I have been able to recover access to production switches with lost passwords using NAC manager. I pushed a switch management policy to the switch and was able to log in via RADIUS. NAC uses SNMP credentials to configure RADIUS on the switch. Glad I was able to recover access but do wonder if that is a security hole.

You can configure what OID is accessible by what SNMPv3 credentials => i do not see it as security hole. You can also backup the config through SNMP and recover it without the admin password... Another way I used...

True SNMP v3 can be secured but it can be done via v2c and v1. I doubt you will find many organizations using only SNMP v3.
