In summary, it is possible that the filesystem has become corrupted.
The worst-case scenario solution is a rescue image of the affected slot(s), but be sure to get a backup save before rebooting anything.

If this is a stack, you may be able to recover semi-gracefully by 'sync slot <#>' on the affected slot while a non-affected slot is the master. That said, you may end up needing to perform a rescue either way.
