Configuring Radius Authentiction

Hi Guys im trying to configure radius auth to my extreme switch stack this is logs i get on the switch.

Slot-1: authentication server for management-access is not initialized

11/01/2017 14:58:23.74 Slot-1: Login failed due to invalid username/password for user luthango.mlityalwa through telnet (10.248.2.64)

11/01/2017 14:58:23.74 Slot-1: Attempted the configured number of retries (3) to each of the 1 authentication servers without a server response for
10.248.2.64(username 'luthango.mlityalwa') on port mgmt.

11/01/2017 14:55:35.72 Slot-1: Login failed due to invalid username/password for user luthango.mlityalwa through telnet (10.248.2.64)

i have used the following commands as the article on extreme suggested:

configure radius mgmt-access primary server 10.248.4.52 1812 client-ip 10.248.111.13 vr "VR-Default"
configure radius mgmt-access primary shared-secret
enable radius mgmt-access

I am not sure what i am do wrong as i have checked the switch can see the windows radius server.

appreciate all help thanks in advance

OK, I am suppose that you are able to ping your radius server and also username and password which are used for communication with radius server are correct!? Could you also please share output of these commands:

sh config | inc netloging
sh config | inc radius
