The pattern of lights that I'm seeing aren't listed in the Installation Manual, so I'm not sure how to interpret what's going on with the AP. Obviously, it's not lovin' life at the moment, but I'm otherwise unsure what's going on. It has persisted across power cycles.
Anyone know what that pattern means?

Interesting; when we swapped in a different AP using the same cable, it came right up, so not an issue with port/VLAN or DHCP/DNS. Might just need a factory reset to get its head on straight.

The LED indicate exactly what you see = no connection to controller.
If you use DHCP make sure the the AP is in the correct VLAN, check if you are able to ping the AP.
How does the AP learn the controller IP…… static, via DHCP, DNS ?!
If you login via the console port you’d check the AP log
# cd /tmp/log
# tail -f ap.log
Or factory reset the AP with # cset factory
