When I run a config no in the bootloader it does not clear the config so I am attempting to reload the firmware
I believe I am following the proper steps to reload the firmware on this switch to default it. Unable to though.
BootRom > download image usb summitX-31.5.1.6.xos
(Re)start USB...

It looks to not like something regarding that USB.
Are you trying to deploy this prior member of a stack as a standalone switch? If so, when you boot in to EXOS are you stuck at "PENDING-AAA"?
If so you may be able to use "REBOOT AS MASTER-CAPAPBLE" as mentioned below.

Also, if you have console access GTAC may be able to assist.
Using the mgmt port and a tftp server like below should also confirm it is something local to that USB:

Went the TFTP Method to reload the Firmware and that worked.
