x460-48p error 5: fan failing / operational again over and over in the log

Hello, Has anyone seen these types of errors? We are seeing these in the log for both switches in a 2-switch stack. Is it really possible that 2 switches are having similar fan problems at the same time? 05/15/2015 10:39:17.80 Slot-2: Slot-2, Error 5: Fan #4 in slot 2 operational. Joshua, I'm not physically at the location, so I'm having my site lead check the LED. The EXOS version is 12.5.4.5 patch 1-4.

What is the physical status of the fan when the failure messages are occurring? What is the status of the fan LED? What version of EXOS? I would also suggest reaching out to GTAC so we can troubleshoot further and identify the issue.
