Cause
When the 'set boot system' command is issued, this immediately affects the output of the 'show boot system' and 'dir' commands - but if the offered system reboot is refused it is necessary to issue a 'clear config', 'reset', or 'configure' command - or another 'set boot system' command with a reboot - before the system will boot using the targeted firmware file. Merely power cycling or otherwise manually resetting the System or individual blades will cause the currently Active system firmware image to be re-used instead.

This is stated in the MATRIX DFE Configuration Guide:set boot system
Use this command to set the firmware image the switch loads at startup.
This is the image that will be loaded automatically after the system has
been reset. Although it is not necessary to choose to reset the system
and activate the new boot image immediately, the CLI will prompt you
whether or not you want to do so. You can choose "Yes" at the question
prompt to have the system reset and load the new boot image immediately,
or choose "No" to load the new boot image at a later scheduled time by
issuing one of the following commands: clear config, reset, or
configure. The new boot setting will be remembered through resets and
power downs, and will not take effect until the clear config, reset, or
configure command is given.

An example configuration sequence is demonstrated below.

This command can optionally reset the system to boot the new image.

Solution
FAD (Functions as Designed)

If after a 'set boot system' command is issued the offered system reboot is refused (indicated when different firmware files are labeled "(Active)" and "(Boot)" in the output of a 'dir' command), consider following up as suggested in the first paragraph above.
